Insurance participation varies by plan. Please verify coverage, benefits, and in-network status directly with your insurance company.

Out-of-Network

Upon request, I can provide a superbill that may be submitted to an insurance company for possible out-of-network reimbursement. Reimbursement is determined by the insurance plan and is not guaranteed.

Standard individual counseling fee: $150 per session

A limited number of sliding-scale openings may be available for self-pay clients. Sliding-scale fees range from $100 to $150 per session and are based on financial need and availability. Any adjusted fee must be agreed upon before services begin.

For clients using insurance, the amount owed is determined by the insurance plan and may include a deductible, copayment, or coinsurance. Sliding-scale fees do not replace insurance cost-sharing requirements.

Good Faith Estimates

Clients who do not have health insurance or who choose not to use their insurance have the right to receive a Good Faith Estimate of expected counseling charges. Estimates are provided when services are scheduled as required by federal law and are also available upon request. To request an estimate, contact Purposeful Path Counseling at 260-368-3091.
